New Student Registration

5^th – 8^th Grade

July 1, 2008 – 10:00 AM – 7:00 PM

July 8, 2008 – 10:00 AM – 7:00 PM

You must have the following in order to register:

·       Original Birth Certificate

·       Original Social Security Card

·       Withdrawal form from previous school

·       Two proofs of residency (utility bill, lease, mortgage, etc.) Must have your name on the bill.

·       Immunization Card (Blue Card)

·       Valid Driver’s license of parent/guardian

·       Custody Papers, if applicable

August 5, 2008 – Homerooms will be posted and you will be able to meet the teacher and bring school supplies – 5:00 PM – 7:00 PM
